Continuation High School Year-end Report 2003-04

 The Year-end Report 2003-04  form, Yr_End_Rpt_03-04.pdf, is a pdf file with an imbedded form.  This means that the form can be filled in either while online or after downloading it to your computer.  After the form is filled in, it can be printed, and then mailed.  The the Year-end Report file requires that the Adobe Reader is installed on your computer.  To get the free Adobe Reader click here.

 These instructions are for the Microsoft Windows Operating System.  Other operating systems may not work the same.  In this case, use the conventions of the operating system to achieve the task.

 Because the Year End Report Form is a pdf file it should work equally as well with any computer operating system that supports the format.

 Step-By-Step Instructions                  [top]

  1. Download the form to your browser from the Internet at http://www.cceanet.org/YrEndRpt/2003-04/YrEndRpt_03-04.htm.

  2. Click the "Downloads" link.

  3. Click "Download Now" next to the phrase "Continuation High School Year-end Report Form" in the download section.

  4. The form should open in your browser.  If it does not, you do not have the Adobe Reader installed on your computer.  Click Here to get a free copy of the reader.

  5. Click in the first field of the form (CountyCode) and type in the information.  You may tab to the next field or click in it. 

  6. Fill-in the form with the appropriate data for the 2003-04 school year.

  7. Fill-in the form as completely as possible.

  8. Make a copy of the form for your records.

  9. Print the form.

  10. Mail the completed form to the name and address on the form by November 30, 2004.

  11. Collect data for the 2004-05 school year to make data collection for next year easier.

  1. Why do I need to install the Adobe Reader?

    The form is an Adobe pdf file.  To read this format you must have the Adobe Reader installed.
     

  2. What is the purpose of collecting this data?

    We are now era where most decisions are based on data.  There is very little accurate data available related to continuation high schools.  This data will be used in discussions with the California Department of Education, State Legislature, Federal Government, and school districts to further the causes of continuation education.
     

  3. Why is the data collection now limited to ten items when previous collections contained many more items?

    Previous data collections targeted data for more than one purpose, e.g. the continuation eDirectory of Schools and teacher contact information.  Also, the CDR decided that the concept of "Less is More" should prevail in collecting data for this effort.  Therefore, the most valuable data that is easily collected was chosen.
     

  4. Must I complete all items?

    We would like to collect all data, however, if you do not know the answer to an item and cannot easily collect the data, send the form with that item missing.
     

  5. Question two asks for an approximate amount, Why?

    We realize that many schools do not compile this data and that compiling it could be difficult.  Therefore we are asking for an estimate.  Please try to be as accurate as possible.
